Which type of medical transport does this patient need?
Do I need a wheelchair van or a stretcher?
If the patient can sit upright for the whole trip, they need a wheelchair van. If they cannot sit upright — after certain surgeries, with advanced illness, with pressure wounds, or with limited trunk control — they need stretcher transport and must travel lying flat. The test is the patient's ability to tolerate sitting for the full journey, not whether they own a wheelchair: someone who uses a wheelchair daily may still need a stretcher for a long transfer, and someone who normally walks may need one after a procedure.
What is a mobility assistance vehicle (MAV)?
In New Jersey, a mobility assistance vehicle is the licensed vehicle class used to carry a patient who remains seated in their own wheelchair. It is fitted with a hydraulic lift or ramp and a four-point tie-down system that secures the wheelchair to the vehicle floor, plus a separate lap-and-shoulder belt for the passenger. MAV is a state licensing term, not a brand or a vehicle model — the New Jersey Department of Health, Office of Emergency Medical Services licenses mobility assistance vehicles alongside ambulances, mobile intensive care units and specialty care transport units.
Is an ambulette the same thing as a wheelchair van?
Yes — in everyday use they describe the same vehicle. “Ambulette” is the older regional term, more common in New York, while New Jersey's licensing language is “mobility assistance vehicle” and most patients say “wheelchair van.” None of the three implies a nurse or an EMT on board. If a provider advertises an ambulette and another advertises a wheelchair van, ask the question that actually differentiates them: what are they licensed to operate, and who is in the vehicle with the patient?
When does a patient need a BLS ambulance instead?
A patient needs a Basic Life Support ambulance when they require clinical monitoring or hands-on care during the trip rather than just accessible seating — oxygen managed en route, a two-person crew for a bed-to-bed transfer, or a condition that could change on the road. This is a different licence from a mobility assistance vehicle, and most New Jersey NEMT companies hold only the MAV licence. Delta Medical Transportation is licensed by the New Jersey Department of Health, Office of Emergency Medical Services as both a Mobility Assistance Vehicle service and a Basic Life Support ambulance service, so the same call covers either level and no one has to re-book with a second company.
Who licenses medical transport companies in New Jersey?
The New Jersey Department of Health, Office of Emergency Medical Services licenses mobility assistance vehicles, ambulances, mobile intensive care units, specialty care transport units and air medical units — more than 4,500 vehicles statewide. It is not the Motor Vehicle Commission that issues this licence, which is a common misunderstanding: a for-hire vehicle registration is a separate requirement and does not authorise anyone to transport a patient in a wheelchair or on a stretcher.
