Eligibility for JACC is based on age (65 or older), income (at or below 350% of the federal poverty level), functional limitations (the participant must need assistance with activities of daily living), and the determination that community-based services are an appropriate alternative to nursing home care. Senior Center Shuttles
Many New Jersey municipalities operate senior center shuttle programs that provide transportation for older adults in the local community. - Frequent, recurring trips are needed — Volunteer programs and shuttle services may not be able to commit to three-times-weekly dialysis transport or daily radiation therapy rides.
- The destination is outside the local area — Senior shuttles typically serve local destinations. For specialist appointments at major medical centers in Newark, New Brunswick, or Hackensack, professional NEMT provides reliable regional transport.
