Arranging non-emergency medical transportation (NEMT) in New Jersey doesn't have to be complicated — but if you've never done it before, the process can feel overwhelming, especially when you're dealing with a health condition at the same time. This step-by-step guide walks you through the entire process, from determining what type of transport you need to what to do if your driver is late.

Step 1: Determine What Type of Transport You Need

The first decision in scheduling NEMT is identifying the right vehicle and service level for your situation. This affects which providers can serve you, how much it costs, and how you request authorization if you're using insurance.

Ambulatory Transport (Sedan)

You need ambulatory transport if you can:

  • Walk to the vehicle with or without a cane or walker
  • Step up into a standard vehicle (car, van, or SUV)
  • Sit upright for the duration of the trip

This is the most common and least expensive type of NEMT.

Wheelchair-Accessible Van Transport

You need a wheelchair van if you:

  • Use a manual or power wheelchair
  • Cannot safely step up into a standard vehicle
  • Need a ramp or lift for vehicle access

Stretcher/Ambulette Transport

You need stretcher transport if you:

  • Must remain in a reclined or supine position during transport
  • Cannot safely sit upright due to a medical condition
  • Have a physician order specifying stretcher transport

Before you call, have the following information ready — this makes the booking process fast and accurate:

  • Your full name and date of birth
  • Insurance information — Medicaid ID, Medicare Advantage member ID, or payment method
  • Pickup address — full address including apartment number, floor, or building name
  • Destination address — full address including suite, floor, or department
  • Appointment date and time
  • Whether you need a return trip — if yes, what time or "call when ready"
  • Your transport level — ambulatory, wheelchair, or stretcher
  • Special needs — oxygen equipment, bariatric requirements, stairs at pickup, need for driver assistance, companion accompanying you
  • Your callback phone number

Step 4: Call or Book Online

Once you have your information ready, contact your NEMT provider:

  • For Medicaid transport — call the transportation number on the back of your NJ FamilyCare card, or call your MCO's member services line. They will coordinate the booking with an authorized NEMT provider.

When you call, clearly state: your name, pickup address, destination, date and time, transport level, and any special needs. The scheduler will confirm availability and provide a booking confirmation.

Step 5: Get a Confirmation

Always obtain a confirmation number or booking reference before hanging up. This is your proof of scheduled transport and is essential if there's a problem later. Also confirm:

  • The name of the transportation company that will pick you up
  • The approximate pickup window (e.g., "driver will arrive between 9:00-9:15 AM")
  • Whether a return trip has been booked and under what arrangement

Step 6: Confirm the Day Before

Call your transportation provider the evening before your appointment to confirm your pickup is on the schedule. For Medicaid rides, confirming with the broker (ModivCare, etc.) or assigned provider is especially important, as scheduling errors do occur. This one step prevents the majority of missed-ride situations.

Step 7: Be Ready Before Your Pickup Window

Be ready to go 15 minutes before the start of your pickup window. NEMT drivers operate on tight schedules serving multiple patients. If they arrive and you're not ready, they may need to continue their route and you could miss your slot. Have your belongings ready, take any morning medications, and be at your front door or lobby when the driver arrives.

How Far in Advance Should You Book?

  • Medicaid NEMT — most MCOs require 48-72 hours advance notice. For same-day bookings, Medicaid transport is generally not available — some plans have an urgent same-day request process, but it's not guaranteed.
  • Private pay with Delta — we recommend 24-48 hours advance notice for planned trips. Same-day transport is often available for private-pay clients depending on scheduling — contact us to check availability.

Setting Up Recurring Standing Orders

If you have regular appointments — dialysis 3x/week, physical therapy 2x/week, weekly oncology — a standing order eliminates the need to book each trip individually. Once set up, your rides are pre-scheduled for every session.

To set up a standing order:

  • Medicaid — your doctor or the treatment facility submits a standing order request to your MCO covering the recurring appointment schedule
  • Private pay with Delta — provide your full recurring schedule and we'll set it up in our system; you'll receive a master confirmation covering all upcoming trips

What to Do If Your Driver Is Late

  1. Wait 10 minutes past the end of your pickup window before calling
  2. Call the transportation provider (or the Medicaid broker) directly — have your confirmation number ready
  3. Call your appointment destination to let them know you may be late — most medical offices accommodate transportation delays
  4. If the driver is significantly delayed or doesn't arrive, contact your MCO if it's a Medicaid ride — you may have grounds for a complaint and reimbursement for alternate transport arranged

Can I get a same-day medical ride through Medicaid in NJ?

Same-day Medicaid NEMT is generally not available through the standard booking process, which requires 48-72 hours advance notice. Some MCOs have urgent same-day request procedures, but availability is not guaranteed. What if I need to cancel my scheduled NEMT ride?

Can I request the same driver every time for my recurring appointments?

With Delta's private-pay recurring schedule, we do our best to assign the same driver to your regular route. This is beneficial for patients who value familiarity, including behavioral health patients, dementia patients, and stroke survivors. For Medicaid-coordinated transport, driver consistency depends on the broker assignment, which is less predictable.

Do I need a doctor's prescription or referral to book NEMT?

For Medicaid transport to most routine appointments, no — a referral is not required to request transport. However, for stretcher transport or other specialized vehicles, most MCOs require physician documentation of medical necessity. For private-pay transport with Delta, no referral is needed.

What should I do if my appointment gets rescheduled after I've booked transport?

Call your NEMT provider as soon as you know your appointment has changed. For Medicaid rides, call the transportation line and inform them of the new date/time.
