Can You Get a Medical Ride Today in New Jersey?
The standard guidance for NEMT in New Jersey is to schedule rides 48-72 hours in advance. But life doesn't always cooperate with advance planning. Patients get surprise discharge notices, urgent referrals come through the same day, or a regular transport provider falls through at the last minute. When this happens, patients and families need to know: is same-day medical transportation actually available in NJ?
The answer is: sometimes yes — but it depends on whether your transport is Medicaid-covered or private-pay, how urgently the trip is needed, and what capacity is available on that day. This guide explains your options clearly.
When Same-Day Transport Is Needed
Common scenarios requiring same-day medical transport include:
- Unexpected hospital or ER discharge — Hospitals sometimes discharge patients faster than expected, and the patient needs transport home or to a skilled nursing facility that day. See our hospital discharge transport page.
- Urgent specialist referral — A primary care physician refers a patient to a specialist who has a same-day opening and the patient can't drive
- Missed or forgotten scheduling — The patient or caregiver forgot to book in advance and the appointment is today
- Prior transport provider no-show — The original NEMT provider failed to show up and a replacement is needed immediately
- Appointment moved up — A specialist or procedure slot becomes available earlier than expected
- Unexpected illness preventing self-transport — The patient normally drives but is too ill to do so today
Medicaid Same-Day Transport: The Rules and Process
NJ FamilyCare (Medicaid) managed care organizations generally require 24 to 72 hours advance notice for NEMT. This is a structural requirement built into the system to allow providers to plan routes and ensure vehicle availability. However, most MCOs have a process for urgent or expedited transport requests when same-day travel is genuinely medically necessary.
How to Request Urgent Medicaid Transport
- Call your MCO's transportation line directly — Do not go through the standard online booking portal for urgent requests. Call and explain that you need same-day transport for a medically urgent reason.
- Explain the medical urgency — Be clear and specific: "My mother was just discharged from St. Joseph's Hospital and needs transport to her home in Clifton today" or "My doctor just called with an urgent specialist referral for this afternoon."
- Request prior authorization verbally — Ask the representative to note the medical urgency in the authorization and to escalate if needed. Get the authorization number before hanging up.
- Have your physician or the hospital social worker call on your behalf — Healthcare professionals calling an MCO often carry more weight than patient calls for urgent authorizations. Hospital discharge planners are experienced in making these calls.
What Medicaid Will and Won't Cover for Same-Day Requests
Even with urgent authorization, Medicaid MCOs may:
- Approve the trip but have difficulty finding an available contracted provider on short notice
- Authorize transport but only for approved destinations (not a non-participating facility)
- Deny same-day requests that lack medical urgency documentation
If Medicaid cannot arrange same-day transport due to provider unavailability, they may authorize reimbursement for a private-pay NEMT trip — ask your MCO if this option is available. Learn more on our insurance page.
Private-Pay Same-Day Transport: A More Flexible Option
For patients who can pay privately, same-day NEMT is significantly more accessible. Delta Medical Transportation can often accommodate same-day transport requests with 2 or more hours' notice, depending on vehicle availability and trip distance on that day.
Private-pay same-day transport works best when:
- You call as early in the day as possible
- The trip is within our service area
- The vehicle type needed (wheelchair van, ambulatory sedan) is available
- The destination is reachable within the driver's available route
To check same-day availability, call Delta directly at (973) 389-3110. Our dispatch team will tell you immediately whether we can accommodate your request and at what time.
Hospital Discharge Same-Day Transport
Hospital discharge is one of the most frequent drivers of same-day transport requests, and it's also an area where planning ahead dramatically reduces stress. If you know a loved one is in the hospital, here are proactive steps:
- Speak with the hospital social worker or discharge planner at admission — ask about discharge transport planning
- Provide your NEMT provider's contact information to the discharge planner
- If discharge is expected within 24-48 hours, book tentative transport even before the exact discharge time is confirmed
- If Medicaid is covering the transport, the hospital social worker can help initiate the MCO authorization process before the actual discharge date
Delta maintains relationships with hospital discharge planning departments throughout NJ and can often be reached directly by hospital social workers to coordinate urgent discharge transport. See our hospital discharge transport page for more.
Tips for Reducing Same-Day Transport Needs
Same-day transport requests are stressful and not always accommodated. The best strategy is to minimize their frequency:
- Book recurring rides as standing orders — See our guide on standing orders for regular appointment schedules
- Set phone reminders — When scheduling a medical appointment, immediately set a phone reminder for 3 days before to book transport
- Have a backup plan — Know which private-pay providers in your area can accommodate last-minute requests
- Build your transport schedule into appointment booking — When scheduling a new appointment, call your NEMT provider that same day to book the ride
- For hospital patients — Engage the discharge planner on day 1 of any hospital stay, not the day of discharge
We serve patients across Passaic County, Bergen County, Essex County, Hudson County, and Morris County.
Does Medicaid ever cover true same-day (less than 24-hour notice) transport?
Yes, in medically urgent circumstances. MCOs have authorization processes for urgent requests, and with a compelling medical reason — hospital discharge, acute care follow-up, physician-documented urgency — same-day authorization can be granted. It requires a phone call to the MCO transportation line (not online booking) and is more likely to succeed when a healthcare professional (social worker, physician office staff) calls on the patient's behalf.
Is there a rush fee for same-day private-pay transport?
This varies by provider. Some NEMT companies charge a premium for same-day or expedited scheduling. Contact Delta directly for current pricing on same-day private-pay requests. We aim to be transparent about all fees upfront.
What if my Medicaid provider can't do same-day and I need transport today?
If your MCO cannot arrange covered transport for a genuinely urgent same-day need, ask your MCO if they will reimburse you for a private-pay trip. Some MCOs have reimbursement processes for exactly this situation. Get the approval in writing (or note the authorization number and representative name) before arranging private transport.
Can a taxi or rideshare substitute for NEMT for same-day needs?
For ambulatory patients with simple transport needs and no special equipment, a taxi or rideshare app may be an option. However, taxis and rideshares cannot accommodate wheelchairs, stretchers, or oxygen equipment, and their drivers have no medical transport training. For patients with any special needs, a qualified NEMT provider is necessary.
What is the earliest I can call to request same-day transport?
For the best chance of same-day accommodation, call as early as possible — ideally before 9:00 AM. Delta's dispatch operates early-morning hours to accommodate dialysis and other early appointment pickups, so calling first thing in the morning maximizes your chances of same-day accommodation.
