Hackettstown Medical Center, part of Atlantic Health System, sits at 651 Willow Grove Street with a 24-hour emergency department, and a Fresenius Medical Care dialysis unit runs up the same street at 657 Willow Grove Street, Suite 202, in the West Wing Medical Plaza. Willow Grove Street is County Route 604; it heads northwest out of the junction where Route 182 ends at US 46, which is signed Mill Street here. Route 182 is a state highway that begins and ends inside town limits, 0.96 miles end to end. For a wheelchair user that concentration is unusual luck — hospital, emergency room and dialysis chair all on one county road, and most appointments never leave Hackettstown.

The gap is everything outside business hours. Warren County Transportation is run for the county Department of Human Services by Easton Coach Company of Phillipsburg, and its demand-response trips go Monday through Friday only, want at least three business days' notice, and stay inside Warren County plus five miles past the border. Every county trip is curb-to-curb, not door-through-door. The fixed-route shuttles do not fill the edges either: the Hackettstown–Washington shuttle's first run leaves at 8:00 a.m. and its last begins at 4:30 p.m., weekdays only, while the Phillipsburg–Washington shuttle runs 6:00 a.m. to 6:00 p.m. and adds a Saturday — Hackettstown gets neither the early start nor the Saturday. NJ Transit's MCM5 and 973 local buses through town were discontinued years ago. So a 6:00 a.m. surgical check-in, a Saturday dialysis chair, or a Sunday discharge from the hospital on Willow Grove Street has no county ride behind it at all.

When Hackettstown cannot handle a case, the referral usually stays inside Atlantic Health System: Morristown Medical Center at 100 Madison Avenue in Morristown, Morris County, or Newton Medical Center at 175 High Street in Newton, the only acute-care hospital in Sussex County. Warren County's other acute-care hospital is St. Luke's Warren at 185 Roseberry Street in Phillipsburg — the county's most populous town, on the Delaware opposite Easton, Pennsylvania — reached west from Hackettstown on Route 57. Morristown alone sits far beyond the five-mile buffer the county van will cross. Post-acute care hides a trap of its own: there is no Medicare-certified nursing home inside Hackettstown town limits. Heath Village, on Schooley's Mountain Road, carries a Hackettstown 07840 mailing address but lies over the line in Morris County, southeast of town.
