Roselle packs about 22,700 people into 2.65 square miles — roughly 8,600 residents to the square mile — and Medicare's provider files list no hospital, no certified nursing home and no dialysis facility anywhere inside it. Roselle Park sits to the north, Cranford to the west, Linden to the south along Route 27, which forms the borough's southeastern border, and Elizabeth to the east along the edges of Warinanco Park. So a hospital visit, a rehab stay or a dialysis chair means crossing a municipal line before the appointment even starts. South of the borough, RWJBarnabas also runs Robert Wood Johnson University Hospital Rahway at 865 Stone Street. Longer runs head to Newark Beth Israel Medical Center at 201 Lyons Avenue in Newark, where the cardiac and heart-transplant programs and the Children's Hospital of New Jersey are. Dialysis is the appointment that repeats, and there is not one chair in Roselle to sit in: patients ride east into Elizabeth — the DaVita unit on Morris Avenue in Elmora, the Fresenius unit on Division Street, Trinitas's own units on Williamson Street — or south into Linden, to the Fresenius center on West St. Georges Avenue.

Roselle's three Union County neighbors to the north, west and south all have NJ Transit rail stations — Roselle Park and Cranford on the Raritan Valley Line, Linden on the Northeast Corridor and North Jersey Coast Line. Roselle has none. The old Central Railroad of New Jersey line through the borough is abandoned, and what is left is buses: the 59, 62 and 94 to Newark, the 112 and 115 to the Port Authority, the 56 and 57 locally. The Union County Paratransit System fills part of the gap and is worth using when it fits, but it runs 7:30 a.m. to 5:30 p.m. Monday through Friday, offers only limited Saturday hours, provides no service on Sundays or observed holidays, schedules out-of-county medical trips on Mondays and Wednesdays only, and asks for five to seven days' notice.
