Hopatcong is served entirely by minor county routes and by the narrow lanes laid out for summer cottages long before the lake became a year-round town. No interstate and no U.S. highway runs through it, and at roughly 997 feet the borough is nearly all grade — driveways that pitch straight down to the pavement, hairpin turns above the water, streets that were never designed around a van with its ramp deployed. Every medical trip out starts one of two ways: south on Lakeside Boulevard, past Hopatcong State Park and over the bridge at Landing, to Interstate 80's Exit 28, the one signed for Lake Hopatcong; or out the southwest corner on Brooklyn–Stanhope Road to Route 183 at Stanhope and Netcong. Picking the wrong one starts the appointment late.

Hopatcong has no hospital and no dialysis unit inside its borders, and neither does most of Sussex County. The whole county has exactly one acute-care hospital — Atlantic Health System's Newton Medical Center at 175 High Street — and exactly one Medicare-certified dialysis facility, DaVita Renal Care of Newton at 7 East Clinton Street. Both are in Newton, northwest of the borough, up Route 183 and Route 206 through Stanhope and Byram. Everything else is over a county line, with the lake itself sitting between the borough's west shore and the rest of it: southeast, past the foot of the water, are Saint Clare's Dover Hospital on West Blackwell Street and, a little further east, Saint Clare's Denville Hospital on Pocono Road, both in Morris County. Southwest, in Warren County, is Atlantic Health's Hackettstown Medical Center on Willow Grove Street.

Roughly one in six Hopatcong residents is 65 or older, in a borough with no hospital, no dialysis chair and no nursing bed of its own — the nearest skilled nursing beds are in Newton and Lafayette, a county drive away. That gap is what a wheelchair van is for. The rider stays seated in their own chair: the driver deploys the ramp or lift at the door, secures the chair with a four-point tie-down, then adds a lap-and-shoulder belt before the van moves. On the shore lanes below Lakeside Boulevard and up through the River Styx section, where a driveway often pitches down toward the road, where the ramp comes down matters as much as when we arrive. Dialysis is the trip that repeats — three mornings a week south to Succasunna or Dover, or northwest to Newton — and it works best with the same driver, the same door and a return time already on the books. Delta runs these from Totowa; there is no depot in the borough.
