The default destination is Community Medical Center at 99 Highway 37 West in Toms River, and the road there is Route 37 itself — it begins at Route 70 in Lakehurst, the borough Manchester completely surrounds, and runs east through the township into Toms River. Past that, the pattern splits by specialty: Monmouth Medical Center Southern Campus on River Avenue in Lakewood to the north, Ocean University Medical Center on Jack Martin Boulevard in Brick further north, and Deborah Heart and Lung Center at 200 Trenton Road in Browns Mills — Pemberton Township, Burlington County — for cardiac, pulmonary and vascular care.

Distance is the local problem. At 81.42 square miles of land, Manchester is the ninth-largest of New Jersey's 565 municipalities by area, and its 45,115 residents are spread across separate named communities rather than one downtown: Crestwood Village, Pine Ridge at Crestwood, Leisure Village West, Leisure Knoll, Cedar Glen Lakes, Cedar Glen West and Pine Lake Park. Most of that population lives around Whiting, on the west side of the township — the side farthest from Toms River. These are retirement communities in the literal statistical sense: at the 2020 census 72.9 percent of Crestwood Village residents and 71.9 percent of Leisure Village West residents were 65 or older, and township-wide 50.2 percent were 65 or older at the 2010 census. Manchester's own western border is the Burlington County line — it touches Pemberton and Woodland townships — so an appointment at Deborah becomes an out-of-county trip that starts inside your own town. Delta is licensed statewide across all 21 New Jersey counties, so that county line changes nothing about the ride.
