It has 52,912 people on one square mile of clifftop — the third-densest of New Jersey's 565 municipalities — and the closest hospital is Palisades Medical Center at 7600 River Road in North Bergen, down at the river's edge below the cliff the town sits on. The vertical distance is the real obstacle. The nearest light rail station sits on 49th Street between Bergenline Avenue and Kennedy Boulevard, just over the Union City line, and its platform is 160 feet below street level in the old Weehawken Terminal tunnel — the only underground platform on the Hudson-Bergen Light Rail, reached by elevator. That shaft is a fair measure of the ridge. Every wheelchair-van run out of West New York begins with a descent and ends, on the way home, with a climb.

Bergenline Avenue is the town's main commercial thoroughfare and its busiest transit corridor, but much of what runs on it is jitneys — private dollar buses that come as often as one a minute, the most frequent dollar-bus route in Hudson County, flagged down from the curb. They are not NJ Transit, and a rider who stays in their wheelchair cannot count on boarding one. NJ Transit's Access Link paratransit covers part of the gap, but it has to be booked in advance and it arrives inside a window rather than at a time you pick. For a dialysis chair time, or a morning appointment on Broadway, that window is the whole problem.

Most West New York care happens somewhere else. DaVita Dialysis at Palisades Medical Center sits at 7650 River Road in North Bergen — for anyone on a three-times-a-week schedule, that is six trips down the cliff and back. Cardiac and trauma follow-up routes south to Jersey City Medical Center at 355 Grand Street, Hudson County's Level II trauma center, or to Hoboken University Hospital. The one substantial medical destination inside the town itself is North Hudson Community Action Corporation's health center at 5301 Broadway, a block off Bergenline. Pickups on Boulevard East mean high-rise lobbies right on the cliff edge; pickups on Palisade Avenue, the town's older commercial street, mean tight curb space. West New York is about three-quarters Hispanic, and Delta confirms these pickups in Spanish routinely.
