There is no hospital on the island. The nearest is Shore Medical Center in Somers Point, reached by the two-mile Route 52 causeway — officially the Howard S. Stainton Memorial Causeway, which everyone here still calls the 9th Street Bridge. The other ways off are the 34th Street bridge on Roosevelt Boulevard to Marmora and Garden State Parkway exit 25; the Ocean City–Longport Bridge north into Atlantic County, a Cape May County Bridge Commission toll crossing that now bills by E-ZPass or by plate; and the Corson's Inlet bridge south to Strathmere. Every one of them is a chokepoint. A 9 a.m. dialysis chair on the mainland and a July afternoon on the 9th Street Bridge are two different trips, and a wheelchair van that has done both plans for it.

The year-round population is small — 11,229 at the 2020 census — and old: the median age is about 57, and more than a third of residents are 65 or older. That mix produces a steady, unglamorous stream of standing appointments. Dialysis riders head up New Road to Fresenius Kidney Care Linwood, past Somers Point. Hospital work runs north to AtlantiCare Regional Medical Center's 323-bed Mainland Campus in the Pomona section of Galloway Township, or south to Cooper University Hospital Cape Regional in Cape May Court House, renamed in July 2024 when Cooper took over the old Cape Regional system. And the island's own skilled-nursing and assisted-living campus, United Methodist Communities at The Shores on Bay Avenue, sends and receives riders all year.

Two local details decide whether a pickup goes smoothly. First, water: the bay-side blocks flood first, and the Simpson Avenue and Haven Avenue corridors are the usual casualties when an onshore wind lands on a higher-than-normal tide — the ocean-side streets like Wesley and Central sit higher. A ramp deployment and a curb approach have to be timed against the tide cycle, not just the appointment. Second, stairs: much of the island's housing is built up above the flood elevation, which puts an exterior flight between the front door and the street, so tell us before the van arrives. Cape May County's Fare Free Transportation is a genuine option, but it is demand-response and first-come, first-served, requires an application, and is run from an office in Cape May Court House open weekdays 8 to 4. Weekend discharges, evening appointments and same-day changes fall to private wheelchair vans.
