Cape May is not on the mainland. The city sits at the southern end of Cape Island, which the man-made Cape May Canal and Cape Island Creek cut off from the rest of the Cape May peninsula, so every medical trip out of town crosses one of two road bridges. One is the Cape May County Veterans Memorial Bridge, carrying Route 109 north to the Garden State Parkway's Exit 0 in Lower Township. The other is the Seashore Road bridge — a 0.7-mile span that is the entire length of the unsigned Route 162 designation — crossing the canal between West Cape May and Lower Township. There is no hospital, no emergency room and no dialysis chair inside the city limits. Add the 380-acre Cape May Historic District, a National Historic Landmark since 1976 with more than 600 buildings, many of them Victorian with high front porches and narrow sidewalks, and getting a wheelchair from the front door to the curb is often the hardest part of the trip.

Anything acute means Cape May Court House, roughly twelve miles up the peninsula, where Cooper University Hospital Cape Regional stands at 2 Stone Harbor Boulevard. It is the only hospital in Cape May County, and it took that name when Cooper University Health Care took ownership on July 1, 2024.

Cape May counted 2,768 residents at the 2020 census, with a median age of 58.3 and close to four in ten people 65 or older. That demographic drives the work: standing three-times-a-week dialysis runs, weeks of daily treatment at the MD Anderson Cancer Center at Cooper radiation oncology suite on the Cape Regional campus, and rides home after a rehab stay. Cape May County's Fare Free Transportation runs Monday through Friday and asks for reservations at least two working days ahead, which does not cover a Sunday discharge or an appointment that moves. Delta runs 24/7. Riders stay in their own chair, secured with a four-point tie-down and a lap-and-shoulder belt; anyone who has to travel lying flat needs stretcher transport instead.
