Transportation Challenges for Pulmonary Patients

Dyspnea with Even Minimal Exertion

For severe COPD or pulmonary fibrosis patients, the walk from their front door to a vehicle can be the most strenuous activity of their day. Our drivers understand that boarding may take several minutes with multiple rest stops. We never rush pulmonary patients. Some patients need to pause at the vehicle door to catch their breath before stepping up; others need to sit on the lift platform for a moment. We accommodate all of this patiently.

Supplemental Oxygen During Transport

Many pulmonary rehab patients are on supplemental oxygen — either continuously or during exertion. Patients typically bring their own portable oxygen concentrator (POC) or oxygen tank for transport. Key considerations:

  • Patients should confirm their POC is fully charged before transport
  • Oxygen tank patients should verify they have sufficient supply for the round trip plus buffer time
  • Inform our dispatcher of oxygen equipment type when booking
  • Smoking is strictly prohibited in our vehicles — this is absolute policy for all patients but critical for oxygen users

Temperature Sensitivity

Cold air and extreme heat both trigger dyspnea in many pulmonary patients. COPD patients are often particularly sensitive to cold air exposure. Our vehicles are climate-controlled and drivers will warm the vehicle before the patient boards in cold weather and cool it appropriately in summer.

Anxiety About Dyspnea

The fear of becoming breathless — particularly in an enclosed vehicle or in an unfamiliar situation — is a real barrier for pulmonary patients. Our drivers are calm, reassuring, and experienced with patients who experience anxiety during transport. If a patient needs to stop and use pursed-lip breathing during boarding, we allow time for that without creating additional stress.

Setting Up Standing Orders for Pulmonary Rehab Transport

Because pulmonary rehab runs on a fixed schedule (typically the same days each week for the duration of the program), standing transport orders are ideal. Once your pulmonary rehab start date and schedule are confirmed, contact Delta to set up a recurring transport authorization covering the full program period. For Medicaid patients, your MCO will need to issue a transport authorization for the rehab program — we can assist with the documentation needed.

Frequently Asked Questions

Does Medicaid cover transport to pulmonary rehabilitation in NJ?

Yes. Pulmonary rehabilitation is a covered Medicaid service in NJ, and NEMT to covered services is included in Medicaid benefits. Coverage details (whether prior auth is needed, how many sessions are authorized) depend on your specific managed care plan. Contact your MCO's NEMT coordinator before starting the program to confirm.

What if I have a bad breathing day and can't make my pulmonary rehab session?

Call us as early as possible to cancel. Pulmonary patients have unpredictable good and bad days, and we understand this. There's no penalty for medically necessary cancellations. Work with your pulmonary rehab team to reschedule missed sessions, as most programs allow some makeup sessions.

My concentrator is heavy. Will the driver help me carry it to the vehicle?

Yes. Our drivers assist with boarding, including helping carry or wheel oxygen equipment to and from the vehicle. If you have a particularly heavy concentrator, let us know when booking so we can ensure the driver is prepared.

Can I use oxygen in your vehicles?

Yes. Patients who require supplemental oxygen during transport may use their prescribed oxygen equipment in our vehicles. We do not allow smoking or open flames in any of our vehicles. Please inform us at booking that you use oxygen so we can note this in your profile.

I finished my 36-session pulmonary rehab program. Can I get transport to maintenance sessions?

Yes, though maintenance pulmonary rehab coverage varies by plan. Some Medicare Advantage plans cover maintenance sessions; traditional Medicare does not. NJ Medicaid coverage for maintenance rehab varies by MCO. Check with your plan and your pulmonary rehab team about whether maintenance sessions are covered and whether transport to them is authorized.
